Hardship letter. Signed and dated.

Last 2 paycheck stubs. If self employed a letter stating income signed and dated

Last 3 moths of bank statements. With your name printed on them.

Tax returns for 2008 and 2009. Only first 2 pages. I do not need schedules.

I will email you a copy of the Financial statement I need filled out.
